About Aios Medical Our overarching mission is to accelerate the mass-market adoption of consumer biotech products so every human can live at least 100 healthy years. Our immediate focus is excess bodyweight. We are increasing the accessibility of GLP-1 medications to accelerate their take-up to help solve obesity worldwide. ClinicOS is an AI-powered clinical operating system that makes it effortless for consumer brands to offer direct-to-patient healthcare globally. We work with external Brand Partners and use this infrastructure to scale our internal consumer healthcare brands. Since launching Fella Health in the US in June 2021, Fella operates multiple brands across the US & UK, with Fella Health and Bolt Pharmacy being the two largest. Our infrastructure aims to serve 100 million patients worldwide by 2035. We aspire to become the world’s first trillion-dollar healthcare company. We have grown rapidly, with revenue over $50M/yr, positive cash flow since June 2023, and over 10,000 customers with high NPS. We are a young startup and still early in our journey; this is Day 1 and much work lies ahead.

Role overview As AI Engineering Lead at Aios Medical, you’ll architect our AI-first future and own the AI strategy across ClinicOS. You will transform how millions access healthcare through intelligent automation, working closely with the Head of Engineering, the CEO, and the AI engineering team to build AI systems that deliver world-class healthcare at software scale. This role blends deep technical execution with strategic vision to ship production AI, define a long-term roadmap, automate clinical workflows, create AI assistants for providers, and scale systems to 100 million patients. If you’re excited about making healthcare more accessible through AI and building reliable LLM-powered apps, this role is for you. This is a full-time, fully remote role requiring adaptability in a fast-paced environment.

Key Responsibilities You will build and lead AI systems across the company, shipping production AI while keeping it simple, reliable, and useful. You’ll collaborate with clinical, growth, and operations teams to make us 10x better. Core AI infrastructure: Build a model-agnostic AI platform that supports OpenAI, Anthropic, and open-source models. Set up robust A/B testing, intelligent routing, monitoring, and reliability with proper fallbacks. AI-powered clinical care: Drive AI adoption across the clinical operating system. Automate manual processes, create AI assistants for the clinical team, and rigorously measure impact on patient outcomes and operational efficiency. AI-powered growth: Transform the conversion funnel with AI-driven outreach and sales agents. Explore tools like AI video generation, voice synthesis, and behavioral prediction; aim for high conversion while reducing manual touch-points. Data & experimentation: Be data-driven, study datasets to identify AI opportunities, build MVPs quickly, run controlled tests with clear KPIs, scale winners, and let data guide decisions.

Requirements Must have 6+ years of software engineering experience with a focus on AI/ML. Strong experience building production applications powered by LLMs (prompt engineering, RAG, fine-tuning, evaluation frameworks). Proficiency in TypeScript and/or Python. Experience designing and building modern, customer-facing APIs and building user-friendly interfaces with React or similar frameworks. Experience leading engineering teams. Nice to have: AI/ML infrastructure experience (vector databases, embeddings, orchestration frameworks); production-grade evaluation and monitoring for LLMs; large-scale data pipelines and distributed systems; enterprise-scale AWS; CS degree or equivalent; open source contributions.
